During the afternoon of October 22, 2022, the Apache server that houses our forms data as well as our helpdesk system experienced a failed upgrade of the underlying server software that forced several sub-sites offline and prevented access to user forms and invoices for our clients. The issue was corrected in short order, but the total downtime amounted to 2 hours and 44 minutes. We apologize for any inconvenience this may have caused.
